Youth Permission Slips

• Individual Event Permission Slip

A permission slip is required for all off-campus events that we transport
youth to. Signed permission slips must be submitted before youth will be
allowed to participate in any off-campus event.

• Blanket Permission Slip

Parents who prefer may sign a blanket permission slip that allows their
child(ren) to participate in any non-over night off-campus event over the
course of a school-year (see dates on permission slip). An additional
permission slip will be required for overnight events.
